SEED Student Survey

New this year, the Oregon Department of Education is requiring all students, grades 3-11 be given the opportunity to complete the SEED survey. For more information, the 4J Seed Survey page is linked here. The survey takes about 15 minutes and will be offered during our 6/12 Advisory class.
 
As with state testing, the SEED survey can also be opted out of. For accurate record keeping, if you choose to decline in writing, please do so with this form, which is also available in the front office, or verbally, in person at the front office, by 6/10/24. If after 6/10, the survey will not be blocked from entry - so please be sure to tell your student not to take the SEED Survey when it is offered if that is your preference.
